The Electronic Vital Event Registration (EVER) system is the state's web-based birth application that allows hospitals, funeral homes, physicians and coroners to register births, deaths and fetal deaths online, thus eliminating the need for a paper certificate.  Starting in January of 2006, this new system will be deployed state-wide and allowed users to easily and efficiently enter the required birth information and submit that information to the Division of Vital Records.  Now, starting in 2008, death and fetal death information will be collected through this application.

This web page is a dedicated resource to all our partners and will provide the required documents, forms and information necessary to use the EVERS application and complete these vital records. . 

Starting on January 2, 2008, all EVERS users will need to access the application at the following location:

https://secure.apps.state.nd.us/doh/evers/main.htm

From this site, you will be required to enter the userid and password you created during the registration process.  If you have problems logging into the application, follow the instructions on the login page for assistance with userid and password problems.

*** Adobe Reminder ***

In order to print the forms in the new death registration portion of EVERS, you will need to make sure that you computer has a recent version of Adobe Acrobat Reader installed on it.  To download the newest version of this software, go to www.adobe.com and click on the link that says "Get Adobe Reader".

*** Hospital Birth Users ***

Please download the latest Parent's Worksheet and begin using it immediately.  The latest worksheet includes the new birth request form and instructions for parents to order their child's birth certificate.  Just click on Parent's Worksheet, under Birth Forms on the right menu.
