Electronic Vital Event Registration System (EVERS)

The Electronic Vital Event Registration (EVER) system is the state's web-based application that allows hospitals, funeral homes, physicians and coroners to register births, deaths and fetal deaths online, thus eliminating the need for a paper certificate.  This system is deployed state-wide and allows users to easily and efficiently enter the required registration information and submit that information to the Division of Vital Records.

This web page is a dedicated resource to all our partners and will provide the required documents, forms and information necessary to use the EVERS application and complete these vital records.  All EVERS users will need to access the application at the following location:

From this site, you will be required to enter the userid and password you created during the registration process.  If you have problems logging into the application, follow the instructions on the login page for assistance with userid and password problems.
