This Guidance provides a general program description and information about coding and reimbursement, training and certification requirements, forms, and endorsements.

All health-care providers wishing to apply fluoride varnish must successfully complete a North Dakota State Board of Dental Examiner's approved training program through the Smiles for Life website.

Smiles for Life Training Completion Form
(MUST COMPLETE after Smiles for Life online training is complete.)
Upon completion of the Smiles for Life online curriculum, you are requested to complete the online training completion form. Submission of the training completion form will comply with the current requirement to apply fluoride varnish under North Dakota Century Code Chapter 43-17-43 for physicians and physician's assistants; and North Dakota Century Code Chapters 43-28-02.6 and 43-28-02.7 for advanced practice nurses, registered nurses and licensed practical nurses.
